Our Process

WISH GRANTING PROCESS FOR THOSE BEING CONSIDERED

• While we always consider the entire wish requested, we may not always be able to accommodate every aspect of the wish.

The Medical Eligibility Form, the nominee’s photo (jpg format) and bio must be received within two weeks of the application submission in order for your wish to be considered; otherwise your wish application will be closed.

The required Medical Eligibility Form must be completed by the treating qualified medical professional and submitted directly from the qualified medical professional's office.

All Wish Referrals are reviewed for eligibility and compliance. • If the nominee does not meet our eligibility and criteria guidelines, you will receive a response within a few business days stating we are unable to consider your request.

• Once we receive all required paperwork (referral and medical eligibility forms, picture and bio) it will take 1 to 3 months for the documents to be processed. Once processed, you will receive a letter by mail informing you if your child has been approved.

• Once approved, we will contact the child’s family within two weeks. If you have any questions, please contact us at 813-330-2555.

• Once you receive an approval letter, your child will be placed on our waiting list. Please be patient and be assured that we are working as quickly as possible to make your child’s wish come true.

• Please be aware A Second Wish By Demetrius does have a waiting list that your child will be placed on if approved. The wait list time begins on your child’s date of approval. You may contact our office at 813-300-2555 to check status of your child’s wish.

• Only immediate family members (i.e. parent/guardian, sister, brother) living in the household; which includes siblings under the age of 18 are eligible for consideration to accompany the wish child. A maximum of (4) four eligible family members will be considered for approval to accompany the wish child.

Currently we are only able to consider travel destinations to Central Florida theme parks.

Due to an overwhelming response to request for second wishes, we are currently not accepting new applications. Please continue to check our website for updates.
